Medical Incompetency tracking

this is not live yet, probably early 2016

Patients who are declared medically incompetent can have much delayed discharges so we want to track when this happens.

Data Collection Instructions

Must have a "Form 21" completed and on chart
- Elaboration: Form 21- The Mental Health Act, c M110(Sub-section 60(1)) - Certificate of Mental Incapacity

Who Fills:
- Must be Attending MD Template:Discussion can anyone else fill this out? Would we not want to know if such other person fills it out? What if it was filled out elsewhere during this hospital stay? Ttenbergen 16:08, 2016 January 11 (CST)
- Psych involved: Not an absolute requirement. Some Physicians prefer not to consult psych but when/if they are consulted it is a written assessment in chart-not on the form. Template:Discussion If its not a requirement, why list it? The patient could also be blond - or not. Ttenbergen 16:08, 2016 January 11 (CST)
- If patient/client already has a POA [Power of Attorney] for any reason and becomes incompetent then the form is not required as they can initiate any/all decisions arising from this determination.-Louise L.
  - Template:Discussion So we would miss in our collection patients for whom a form was not filled out because a POA exists? Don't a lot of people have one filled out just in case? Doesn't there still have to be a determination of some kind that makes it come into force? Ttenbergen 16:08, 2016 January 11 (CST)
