• Deer Lodge - except PCH part, code that as PCH
• Riverview - except PCH part, code that as PCH
• St. Amant
• any hospice, though these should obviously be rare
• Selkirk Mental Health Centre

For Previous Service - enter NA / not applicable for direct admissions from chronic care facilities.

If a patient's discharge to a Chronic Health Facility is delayed, you might be able to code Awaiting/delayed transfer to long-term care/PCH inside or outside of Winnipeg.
