A. If NEW position:

  • SAP ESF (Staff RX) email to: Admin assistance (Terri Kolody) for VP/CNO (Lori Lamont) for a signature to approve posting of NEW position.
    • When you email the ESF to Terri, in the email ask her to let you know by email when she has forwarded ESF to Finance.
  • SAP ESF is the forwarded by email to WRHA Finance for budget approval- designated contact (July 2013 - Kim Kummen).
    • When you email finance with ESF, they must email you back with ESF attached and email indicating can proceed to EmploymentServices WRHA (in global address book).
    • IMPORTANT: Make sure to email EmploymentServices to remind them to posts these jobs with following pre screening questionaire:
  1. Are you currently registered with the CRNM? If not, are you eligible for registration?
  2. Do you have two (2) year of recent ADULT intensive care nursing experience?
  • ESF is forwarded on the payroll by EmploymentServices department. Payroll Representative for our cost centers are: Salvie Quintos,(Anna Placido-Honke: Manager).Trish Ostryzniuk 11:07, 2013 July 31 (CDT)

B. If filling vacant position:

  • Complete SAP ESF for employee leaving position
  • send ESF by email to: EmploymentServices WRHA (in global address book).-

WRHA stuff

FOR WRHA HR -650 Main St. 3rd floor:

  • SAP ESF - Employee Status Form (ensure proper payscale step, hourly rate, vacation (accural rate & if transfer from other site, number of days remaining that manager must schedule before end of fiscal year - March 31).
    • SIN #
    • DOB
  • CRNM Registration # so you can check if currently registered online.

Also must be submitted along with ESF for NEW hires (not transfers within our program):

  • copy of interview and score card
  • copy of
    • degrees
    • diplomas

Education certificates relevant to position (ICU course)

  • Criminal Record Check (must be current within 6 months if new employee)
  • Adult Abuse Registry Check. (go in person to apply, obtain receipt, submit receipt to employer
    • Bring two pieces of ID. One must include a photo. (Not MB health card or SIN card).
  • the above two checks will be submitted to employer within 4 weeks of start date of work.

        • Police station or Public Safety Bldg
        • wait time 2-6 weeks
      • If for some reason there will be a delay to get a criminal record check done then you must "sign a Oath, with WRHA that is good up to 4 weeks to before obtaining the actual record check. Information can be found on WRHA Insite in regards to this. If record check is not submitted to your employer within 4 weeks of employment start date, your employment can be terminated.Trish Ostryzniuk 17:11, 2014 March 3 (CST)

Other Forms & things

  • fill out FORM for WRHA ID badge. Manager must sign before ID obtained. See: WRHA photo ID information WRHA website
  • To obtain ID badge bring signed form to 496 Hargrave only on Fridays between 1-3 PM.
  • fill out site specific FORM site swipe card where applicable
  • fill out site specific FORM for office key
  • notify site manager where individual will be assigned.
  • organize orientation training with main office and then partnering scheduling.
  • staff to arrange parking on their own with the site assigned to.
    • STB: Phone: call 237-2319 and be placed on a waiting list.

IT Support

  • computer accounts eHealth ServiceDesk - Go to ehealth service catalogue and fill out a eHealth service request FORMS. Send to electronic form by email to: Manager for signature.
    • refer them to Sandy Kroeger's instructions for NEW hires or for staff transferring into our WRHA program from another WRHA program
      • met with reps from the CUE project, this will modify Sandy's instructions over the next few months Ttenbergen 22:46, 2012 November 14 (EST)
    • network login access
    • access to file share on Regional Server
    • add RIS/PACS account
    • site specific EPRs, see Category:Hospital EPR
      • set up EPR training for those to work at STB. Classroom training A is required.

  • DC CC (Data Collection Critical Care)
  • DC Med (Data Collector Medicine).
  • Also subgroup mailing list for each Site.
