The [Green Sheet] is a bright green colored paper form that is used in Critical Care and on Medicine wards in the Region and it's purpose is to obtain "required" participation in the Database Program from the attending physician for the following:
- primary admitting diagnosis
- complications (problems, procedures and surgery) that occur after unit admission
- Comorbid conditions
- Admit, Transfer and Discharge date and time
- Go to greensheets.mdb for more information.
It is the data collectors responsibility to remind site attending physicians to complete these form.
