The green sheet is used in Critical Care and on Medicine wards in the Region and it's purpose is to obtain "required" participation in the Database Program from the attending physician for the following:
- primary admitting diagnosis
- complications (problems, procedures and surgery) that occur after unit addmission
- comorbid conditions
- admit, transfer ready and discharge dates and times
- The Data Collector in Medicine & STB ICU's collect information on the compliance rate for the completion of these forms. Since there are inconsistencies in filling these out, we are compiling stats about their completion in the L_Green table.
