*in your outlook go to public folders
*select all public folders
*go to Security Services folder
*See Facility Access form
*must be signed by: [[p:Trish Ostryniuk | Manager]]

To get the forms: 
*Go to WRHA intranet.  *Select "STB intranet" on the main page.
*Select "forms",  
*Select "Protection Services/Security",
*Select desired forms (Access card or key requisition).

Local swipe card for facility access

HSC

To get the form:

  • in your outlook go to public folders
  • select all public folders
  • go to Security Services folder
  • See Facility Access form
  • must be signed by: Manager
    • Room MS245
    • M-T 9-3, Fri 9-12

STB

To get the forms:

  • Go to WRHA intranet. *Select "STB intranet" on the main page.
  • Select "forms",
  • Select "Protection Services/Security",
  • Select desired forms (Access card or key requisition).
