At HSC, pharmacy data is manually collected by the ICU PHARMACY DEPARTMENT.

Note: For information about pharmacy data collection in ALL OTHER ICU's ( STB,GRA,OAK,CON & VIC) in the Region, go to: Pharmacy Collection Other Sites or go directly to S_Drugs for list of drugs that are on the PDA for these sites.


Processing of the Pharmacy Data

The Pharmacy data from HSC ICU is manually entered irregularly into the Critical Care Database by the data processor. Data collectors at HSC do not collect any pharmacy data. The HSC Pharmacy department have participated in this part of the collection process since mid 1988.

Requesting of HSC Pharmacy Data

  1. HSC pharmacy requests a patient TMSX Registry log (Logprnt.exe) Program list from Pagasa or Trish when ever they are ready to look up the pharmacy records.
    • Registry Log (Logprnt.exe) Program list includes the following: Study, serial, LN, FN, DOB, Chart, Admit date, Admit Time, Discharge date, Discharge Time, unit, (enc) encounter, site, PHIN, Prov, Sex.
  2. once their collection of pharmacy items is complete they drop the HSC Pharmacy Collection Forms off in the data processing office.

Entering of HSC Pharmacy Data

  1. Make the secondary PC Pharmacy.dat the Master.
  2. Secondary PC enters paper pharmacy collection form into TMSX pharm.dat.
  3. Make main data processing PC Pharmacy.dat Master again.

  • Historically ICU pharmacy was part of the HSC ICU QA Team when this database program first started at HSC in June 1988.

Pharm Output Errors -Period July 7 to Sept 14.09-CCMDB.mdb output errors

  • when new laptop (netbook) version of Access CCMDB.mdb was rolled out and first sending starting July 7.09, specified pharmacy items that should have been divided by base amount before output had not programmed into this version of AccessCCMDB.mdb. For example Pharm item 90-Heparin, if given collected as 2 days of 7500 unit per day, this should be output as 3 doses of 5000 not 15000 units. The Heparin error was fixed in the database. This error occurred during period when collection staff were on short noticed, redeployed to bedside for H1N1 and coincided with the roll out of a new version of ccmdb.mdb. Every drug item that have been affected have been Id'd a fixed up as of Jan 20.10 as per Julie excellent worksheet that help us conquer the problem.--TOstryzniuk 12:55, 15 October 2009 (CDT)
