Causes and solutions

1. look up the PHIN number on the scanned form image
2. go to CFE, look up the patient with that PHIN and use the Cat button to find related records
3. check if the to and from fields tell you that this was a wrong location, confirm via Finding a patient in EPR what the location should be
4. check the serial number part of D_ID to see if one is similar to the one on the TISS form; if so, follow Fixing a D ID in TISS28.accdb and change the TISS D_ID to the one from CFE
