* Fax Lorna Sasley at 75002 or take a print out of the medical record request to her inbox at her desk in medical records.
* Email Lorna Sasley or take a print out of the medical record request to her inbox at her desk in medical records. Use the [[MR Organizer form]] and the [[MR List]] to generate this list in CCMDB.  Charts marked MR* have not been requested, MR signifies that these charts have been requested and there should be a corresponding printed list to go with them.
* charts requested before 1400 hrs will be pulled by MR staff overnight and some should be available the next day as a general rule of thumb.
* Charts have to be assembled before being pulled, and depending on the backlog in MR, can take a few weeks before being available.
* if charts are from iron mountain, they will take 2 days.

This article provides information on how to request patient charts from Medical Records at HSC. It is part of the HSC General Collection Guide.

Requesting charts

  • Email Lorna Sasley or take a print out of the medical record request to her inbox at her desk in medical records. Use the MR Organizer form and the MR List to generate this list in CCMDB. Charts marked MR* have not been requested, MR signifies that these charts have been requested and there should be a corresponding printed list to go with them.
  • Charts have to be assembled before being pulled, and depending on the backlog in MR, can take a few weeks before being available.
