A nursing home ward (E4) is being set up temporarily at Grace Hospital to host patients that should be discharged to a nursing home but for whom there currently isn't space.
- Collectors will code as discharged to or admit from as: Nursing Home Grace in CCMDB.accdb
Start/End Date
- start: 2014-01-23
- end: Unknown, last entry 2017-10-09
HAP dx allowed
patients admitted from this ward can be coded as having HAP-Hospital Acquired Pneumonia
Pre-admit Inpatient Institution field/readmission
- We consider admissions from Grace Nursing Home Ward as readmissions, so would be coded as if they had come from home. Consistent with that we will consider these not inpatient locations, so code them as "NA / not applicable". (As per meeting with Dr Roberts 2016-05-05)
background
with a projected bed capacity of 12-15.
- Duration of opening is expected to exceed one and a half years. Unit was designed to reduce the pressure of opening contingency beds on all 3 Medical Units which have been running at numbers exceeding capacity since the beginning of January 2014. "These patients will be considered discharged so far as the database is concerned and they become a new admission if they bounce back." (as per Dr Roberts). Code these patients as discharged to "Nursing Home Grace" (will be available as of Mon 2014-Jan-27), and treat admissions from this ward as new admissions.
