This article provides information on how to request patient charts from Medical Records at HSC. It is part of the HSC General Collection Guide.
Requesting charts
- Email Lorna Sasley or take a print out of the medical record request to her inbox at her desk in medical records. Use the MR Organizer form and the MR List to generate this list in CCMDB. Charts marked MR* have not been requested, MR signifies that these charts have been requested and there should be a corresponding printed list to go with them.
