* CTU S4 Medicine data collector enters EMIPs on CTU S4 laptop.

* Go to Medical Records and ask for the Monthly Admit/Discharge binder**(Bed Census Services Report. Locate patients who have been admitted and discharged from the ER under CTU/VMU (Medicine) Service(Medicine Service physician list available in Data Collection Office). Discharges will be either home or transfers to another facility. Jot down patient name and MRN chart number along with the discharge date. Give list to medical record staff and they will pull the charts for you.


The EMIP binder with serial log numbers is kept in the Data Collection Office on the shelf by the door. Enter EMIPs onto CTU S4 laptop. When reviewing charts confirm patient was admitted to Medicine Service. Since EPR the paper patient record will be incomplete as ER staff do not print off record sheets; EPR will need to be accessed to obtain missing information: ie VS, admit date/time to CTU/VMU, etc.

Check the Admit/Discharge binder every couple of weeks and initial pages that have been checked, so the next collector is aware, alternately indicate a date in the EMIP binder located in the office.
