River Ridge Transition Care Environment (TCE) River Ridge Transition Care Environment (TCE)

Patients transferred to the TCE are medically stable and ready for discharge from hospital, but still need time to make long-term arrangements for care. In the TCE, the care team assesses and determines the level of care and support necessary for each patient.

Following a TCE stay, patients may be able to return home with home care supports, be eligible for supportive housing, or PCH.

River Ridge can be collected as Previous Location or Dispo.
