Teleform

Teleform is the software used by the data processor to scan the TISS Forms before importing into the Critical Care Database repository TMSX. We are currently using Teleform v8.2.

Hardware

• see Scanner
• Version 8.2 requires a USB key that is plugged into the back of the scanner PC

Install media for Teleform

The install media for Teleform is located in the left, far-back cabinet in the filing area.

Installing Teleform

• Enter CD, follow wizard. Note: Teleform will install
  • Pervasive 2000i
  • an ODBC source (hmmmm Ttenbergen 16:31, 29 June 2011 (CDT))Template:Discussion
  • ZEON DocuCom PDF Driver
• Set up Scanner
  • Open Teleform Reader
  • Menu Scan, Launch Scan Station...
  • In Scan Station, menu File, Scanner Manager
  • "New" button
  • Interface --> "Twain"
  • Button "Select", choose our Scanner
  • click OK
• Restore the Teleform form for TISS
  • In Teleform Reader, menu File, Import a Form
  • browse button, navigate to backup directory, likely C:\Teleform_form_backup
  • Choose the form V6Tiss34, OK
    • NOTE: to find the actual TISS form in which was designed in Teleform, you must launch the Teleform Form Designer Program on the PC where the form was installed before.

Legacy Info

Teleform was also used to scan in iTISS forms for the Medicine Database Program MedTMS. This was stopped Dec 16.2006.
