- Na+ < 120 OR
- Any Na+ < 135 AND in the presence of signs or symptoms believed to be due to hyponatremia OR
- Any Na+ < 135 with active treatment for hyponatremia

- Main signs and symptoms of hyponatremia are:
- CNS - altered sensorium, seizures, coma
- GI - anorexia, nausea/vomiting (these occur early)
- neuromuscular - muscle cramps, hyporeflexia
