Collection start: within the next few weeks "within the next few weeks" contains an extrinsic dash or other characters that are invalid for a date interpretation.

Data Collection

For any medicine record, capture when the patient is medically stable and ready to leave an acute medicine bed.

Data Entry Instructions

The following lines will automatically be added to each new medicine record:

  • Project: MR - Item: "not entered" (because you haven't set a value yet)
  • Item: change the item dropdown as follows as you come across information
    • "ready" - physician documented that patient became ready at least once (enter dttm)
    • "never" - physician never documented that patient became discharge ready before dispo (no dttm)
    • "not documented" - data collector has strong impression that pt was ready to leave but no physician note (dttm if available)
  • Date: date as described in "item"
  • Time: time ' '
  • Integer: not used
  • Real: not used
  • Checkbox: not used
  • Comment: not used
