Collection Instruction

For each patient enter the date and time when the patient left the unit as written in the unit logbook, EPR, nursing note or chart whichever is more accurate at your site.

  • Con and I have been discussing our transferred patients from one medical ward to another via DIALYSIS UNIT; Laura commented on the fact StB admits patients FROM dialysis units-I am understanding that we cannot discharge to dialysis-especially if patient is being transferred to another ward.We have previously coordinated times from receiving unit arrival-IE: Con gets patient at 1500hr from HD-I set my transfer to A4 At 1450h from B3.
Con would take the dialysis time but I would be sending patient TO dialysis 3-4 hours earlier-at present-AT HSC- the patient is under care of prev ward until arrival at following ward-how does our management team advise our practice at HSC???--Llemoine 15:12, 2017 April 3 (CDT)

We track Dispo DtTm field for patients as part of our Minimal Data Set, i.e. this his high-priority data that needs to be collected ASAP.

discharge at midnight

If the patient is discharged at 24:00/midnight, use 23:59 hours as the discharge time. This is so that Minimal Data Set can check for date being entered properly even if time is not yet available.

Expired

Use time of death as Dispo DtTm. However, for Organ Donors patients see that article.

Legacy

This field is part of the 2016 Time and Place changes. Similar to the old Discharge date and time resp the fields L_Log.R_DisDate and L_Log.R_DisTime.
