Hiring a New Data Collector

A. If New position:

    • submit staff Requisition (Staff RX) to Admin assistance (Terri Kolody) to VP/CNO (Lori Lamont) for signature approval of new position. (June 2010)
    • Staff Rx is the forwarded on to WRHA Finance for budget - designated contact (June 1.10 - Hong Wei)
    • forms are forwarded on WRHA HR - designated contact - (Sept 22.10 Danielle Lafond)
    • forms are forwarded on the payroll which is at the HSC. Payroll Representative for our cost centers are: Inka Calvert as of Sept 22.10

B. If filling vacant position:

    • Complete ESF of employee leaving position
    • fill out a staff Rx sent to WRHA HR.


  • computer accounts - eHealth ServiceDesk. - fill out an electronic request FORM
    • network login access
    • access to Share Point on Regional Server
    • if STB - fill out a Site specific FORM for access to ERP (Electronic Patient Record). Sign by site Director. STB-eHealth application form
      • book ERP training with STB site. FORM from STB
    • If HSC - access to Delphic RTS
  • new laptop if for a new location
  • add to Global address Group mailing list: DC CC (Data Collection Critical Care), DC Med (Data Collector Medicine). Also subgroup mailing list for each Site. Mailing list manager: Trish Ostryzniuk
  • set up wiki account Setting up a new wiki user
    • FOR WRHA HR -650 main st 3rd floor:
  • ESF - Employee Status Form
  • copy of interview and score card
  • Criminal Record Check (must be current within 6 months if new employee)
  • copy of CRNM
  • copy of degrees, diplomas and Education certificates relevant to position
    • Other
  • fill out FROM for WRHA ID badge
  • fill out site specific FORM site swipe card where applicable
  • fill out site specific FORM for office key
  • organize orientation training with main office and then partnering scheduling.
