A. If New position:

  • SAP ESF (Staff RX) to Admin assistance (Terri Kolody) for VP/CNO (Lori Lamont) for a signature to approve new position. --TOstryzniuk 16:38, 28 June 2011 (CDT)
  • SAP ESF is the forwarded on to WRHA Finance for budget - designated contact (June 1.10 - Hong Wei)
  • forms are forwarded on WRHA HR - designated contact - (June 2011-Ivy Hung -HR assistant - Nick Pasquarelli -Senior HR consultant.)-Trish Ostryzniuk 20:26, 2012 November 7 (EST)
  • forms are forwarded on the payroll. Payroll Representative for our cost centers are: Salvie Quintos,(Mark Hamer-Manager).Trish Ostryzniuk 20:26, 2012 November 7 (EST)

B. If filling vacant position:

  • Complete SAP ESF for employee leaving position
  • send ESF to: WRHA HR (Ivy Hung)-Trish Ostryzniuk 20:26, 2012 November 7 (EST)

IT

WRHA stuff

FOR WRHA HR -650 Main St. 3rd floor:

  • SAP ESF - Employee Status Form
    • SIN
    • DOB
  • copy of interview and score card
  • Criminal Record Check (must be current within 6 months if new employee)
  • CRNM Registration #
  • copy of degrees, diplomas and Education certificates relevant to position

Other Forms & things

  • fill out FORM for WRHA ID badge
  • fill out site specific FORM site swipe card where applicable
  • fill out site specific FORM for office key
  • notify site manager where individual will be assigned.
  • organize orientation training with main office and then partnering scheduling.
  • staff to arrange parking with site assigned to.
