New Pt Serial helper form

The New Pt Serial helper form starts a new record in CCMDB.accdb. It is opened either by the "Add Patient with Serial Helper" button or the "+" button on the Cognos2 Service Starter.

Instructions

The Pat ID/Serial number will be populated with the next higher number than the highest Pat ID/Serial number.
