* When info is available at another site, we should link to it instead of copying it. (a) because copyright, and (b) because that way if they edit we get that edit as well, and if they remove the page (ie something changed) it will show up in our broken links report (see link below, it breaks discuss template). So where was this from?  Ttenbergen 11:38, 2022 October 19 (CDT) Thanks Tina, the link is included below.
** The only TC unit dispo locations currently available are for Misericordia and VGH. Deer Lodge as a dispo could indicate either PCH, chronic care, or rehab units. There is only an OAK ward as dispo option and could mean any number of different services including geri rehab. Same for Riverview dispo location could indicate PCH, palliative care, chronic care, or rehab. Is additional detail desired or needed for dispo locations to include TCU's? Should this be discussed at task? Thank you
* I'm not sure if Julie is using this data we can ask.  For Deer lodge and Riverview if the patient is discharged to the PCH ward of that location then we are to put the dispo as Winnipeg, PCH,  Lisa Kaita 11:14, 2022 October 20 (CDT)
** Just wondering whether this is confusing or counterintuitive as there are actual Deer Lodge and RHC dispo locations, especially for new collectors. In the Chronic Health Facility article it only speaks to admissions from PCH part of Deer Lodge (and RHC) however there is no reference to Deer Lodge and RHC as a dispo location. If the pre-acute living situation is coded as PCH, then does it matter that the patient is discharged to Deer Lodge or RHC as a dispo location in these instances? Is this detail desired; should we consult with Julie?
* We have discussed this at TASK in 2019  here is the final decision that was made - (Long discussion about if/how to split up the different types of beds at Riverview and Deer Lodge. Specifically, there are PCH beds and non-PCH beds. At Riverview the latter include chronic vent beds, rehab beds, and palliative care beds. After discussion we agreed that the only distinction we will have in those 2 places will be between the PCH beds and all other non-PCH beds, which we will call “Chronic Health Facility” beds. Likewise, St Amant, Selkirk Mental Health Centre and hospices will be considered to be Chronic Health Facility beds. Chronic Health Facility was updated to reflect this.)  I will update it to include that when discharged to a PCH bed at Riverview or Deerlodge that the correct dispo location would be Winnipeg, PCH  I will also double check with Julie.  Lisa Kaita 10:46, 2022 October 21 (CDT)

Transitional Care is provided in Transition Care Units (TCU)s.

A transitional care facility is a temporary place for patients that are waiting for placement into an alternate care setting.

Other medical care and rehabilitation is provided before going home, to a nursing home or to other more permanent placement. This includes Geriatric Rehab.

In the WRHA setting TCUs usually have a defined period of time for the resident's stay to determine most appropriate final discharge destination.
