This dataset contains data in support of the LAU Collection Project.

Home care and PCH admissions suspected to be factors in long LOS on LAUs. We need to find out the time between consult and Dispo_DtTm find out if that is so.

Data Collection

  • For any record collected as part of LAU Collection Project, enter the date and time of the first Homecare consult and the first Panelling request for each admission to an LAU

Data Entry Instructions

For every LAU Collection Project, a line with Project = "Consults" and item = "not entered" will automatically be entered. Update that line to one of the following:

  • Project: Consults with one of
    • Item: "Homecare"
    • Item: "PCH Panel"
    • Item: "no consult" (cross-check item if there were no consults)
  • Date/Time:
    • dttm of the consult
    • first Service tmp entry DtTm if patient arrived with consult in progress
  • Comment field was used to make some general notes about some consults
  • Checkbox, Integer, Real: not used

Sources

  • Orders
  • consult section
  • progress notes
  • EPR ER notes/documents

Data Use

This data is being collected as a current state for LAU Collection Project.

No general data quality issues found. There is at least one entry for each record in the LAU Collection Project. Where there is more than one, it is always a pair of "PCH Panel" and "Homecare".

Home Care Documents Late

The data collector noted:

  • Often HC is consulted very early, but they don't actually document in the chart until very close to discharge, she assume they are following these patients, likely during rounds, and waiting until they are medically stable.
  • The item PCH panel, is interesting because the team often knows they need LTC but cannot proceed to panel without the approval of LTC, and getting the approval of LTC can be a lengthy process
    • This comment relates this data to the LAU LTC consult data we also collected
