Obesity-hypoventilation syndrome (Pickwick syndrome) is a subset of Sleep apnea, obstructive, in which in addition to having apnea with episodes of Hypercapnia (hypercarbia) during sleep. These people also have hypercapnia while awake. As a result, these chronically hypercapnic patients generally have compensatory metabolic Alkalosis (i.e. high serum bicarbonate to counteract the effect of the high pCO2 on blood pH).
- The diagnosis is generally made based on:
- demonstrated sleep apnea,
- Obesity, NOS (BMI 30.0 to 39.9) or Obesity, morbid (severe, BMI gte 40),
- and daytime hypercapnia not explained by other disorders (e.g. severe COPD).
You can include Obesity-hypoventilation syndrome (Pickwick syndrome) as one of the last/lowest priority Admit Diagnosis to capture this problem even though it is not reason for admission.
While we did not allow sleep apnea as a Comorbid Diagnosis in the old codes, this code will be allowed.
