I've never have done the transfer ready time this way for ICU.''' Is this new?''' We have lots of patients go ACP-C and die shortly after do you really want a transfer ready time in this case? Anyone do you know? I have used the transfer ready for those that had comfort care and were transferred to the ward but not the ones in the unit that die shortly after they become ACP-C[[User:GHall|GHall]] 16:57, 2015 June 4 (CDT)

**I agree, I haven't been including a transfer ready date for those patients who are changed to ACP-C and end up dying in the ICU. The only time I would add it is if patient lives long enough to be transferred to a medical ward, or if a transfer order is written in the chart.~~

If a patient is on the transfer ready list in ICU and his '''condition deteriorates''' resulting in the need to remain in the unit, then this patients transfer ready is '''cancelled''' and the transfer ready date and time should be deleted. If the patient is put back on the transfer/discharge ready list, enter the '''new''' transfer ready date. '''Not applicable to Medicine ward patient.'''
