ICU
- These are meant to be criteria to consider in cardiogenic shock. It is understood that not all patients will fall nicely into all listed criteria. Remember, use your judgement.........
- the patient is in shock -- i.e. substantial absolute or relative hypotension and/or other evidence of systemic hypoperfusion (such as elevated serum lactate).
- there is no evidence of septic, hypovolemic or distributive shock or if these are present, there is also evidence of cardiac insult.
- parameters to consider for diagnosis of cardiogenic shock:
- Definite pulmonary edema
- May require ventilation for pulmonary edema.
- May require IABP
- CI < 2.2
- PWP > 20
- Requiring inotropic support
- Ejection fraction <35%
- Current diagnosis of ACS or cardiomyopathy
- Current diagnosis of severe cardiac valve disease.
Medicine ward
- Is not normally managed on a medicine ward therefore we would not see it as an admitting diagnosis. Cardiogenic shock can be a complication that occurs on a medicine ward which usually requires a transfer to the ICU unless the patient receives palliative care or dies before being sent to the ICU.
