This is the contact list for the CCMDB. Click on the sub-category Offices to see a listing by location.
The most current Contact information will only be updated here WIKI & in the Global Email Address Book. We will no longer be sending out any paper contact list. Please discard any out-of-dated information at your site. Further, please don't print this out so you don't end up with out-of-date information.
On the left side of this screen you will see "search". In here you can type in the data collectors name and find them on the WIKI. Even if you just type in the first name, you will be able to find what you are looking for. You can search for hospitals and wards as well, but keep in mind that the shortest term you can search for is 3 letters.
Outlook Global Address Book
We have mailing lists for all programs and locations in the Outlook Global Address Book. The names follow the convention DC_<hosp>_<program>, e.g. DC Vic Med.
- there are 12 contact groups for our program.
- DC CC - contains all the members of the ICU collection group in the city.
- DC MED - contains all the members of the Medicine collection group in the city.
- to look into a DC contact group to see "who" is a member of the group:
- Open an email
- in the "TO" field, type: DC. You will see all the DC contact groups that we have.
- click your mouse "once" on the group you want.
- right click your mouse on the contact group and it will open the group allowing you to see who are the members.
- to sent to a DC group:
- open email
- in the "TO" space, type: DC. You will see all the DC contact groups that we have.
- double click you mouse on the group you want. This will put it into the "TO" field
- press OK, and you can now type your email and send to all collectors in the selected contact group.
- Open an email, put the location in the "to" field and tab out of that field. A small "+" will appear to the left of the name. Click that "+" to expand the list into it's individual collectors.
Discussion viewing members in contact group via Webmail

Template:Discussion Contact groups - collectors cannot view members who are in a DC contact group in the global address book via webmail. This is not helpful to collectors. Tina, are there any suggestion how they can see who is in a contact group? the instruction above does not work.--GHall 12:22, 8 June 2011 (CDT)

Outlook Web Access
Template:Discussion The plus will not work in Outlook Web Access. Collectors logging in as themselves anywhere including the laptops should now be able to use regular Outlook. There should be a shortcut under Start menu -> programs -> microsoft office -> microsoft office outlook 2003. If this works we can set up a better shortcut. Please try and let me know. PHIA Warning
Do not use contact function on this wiki to send any emails that contain any patient identifers such as name, chart, PHIN.
The wiki is outside the WRHA network. Because it facilitates the email, it gets to see what goes into the email. So, it must not be used for patient data. I realize this would have been very handy, so I am sorry that we can’t do it that way. Having said that, we might need to look into a better way for you to share some of this data.
PHIA Concern with broadcast email
However, putting into place a solution that automates our sending of personal health data to a broad list of people in hopes of it getting to the person who needs it would go against PHIA. Sending emails with patient data inside our network to specific people who need it is within the spirit of the law. Sending it to a crowd to find the right person would not meet PHIA.
Interchange
If this is the main reason we want this functionality, then lets instead fast-track the implementation of a different way to deal with pending patients: Interchange
