  1. Admit Type for APACHE II. The Centralized import from Ed.mdb possible conversion problem. Tina needs to check the coding of elective surgery, emergent surgery, medical. Imported, data not matching what is in Ed. Julie could advise further.
  2. Centralized - the diagnosis code for ACUTE PERITONEAL DIALYSIS (TASK) should be changed from 1003201 to 100301. Julie Mojica as per email sent April 30.14.
  3. ED’s has 8 missing CH data (due to missing APACHE elements – same with ES)Chronic Health APACHE. Centralized_data - assigns data to the missing 8 cases. These should have been treated as MISSING in the centralized data. Julie Mojicaas per email sent: April 30.14.
  4. adjustments (in bracket) to APACHE Scoring table done on APRIL 23.14 (see edit history). Would need to check and update centralized front end.

  5. Sending to centralized data.mdb - Record Status field changes in centralized that seem to occuring when send files are uploaded to centralized.mdb. For some reason random files going into centralized, and Record Status field is changed to SENT, yet files had not been sent as complete to CVS for that date. They remain as Incomplete in record status field on their laptops. Collector appear to be able to send file to CSV the following week, but we have not heard of error message that status is already SENT in centralized and they could not resend. Sites affected: HSC: A4, D4, H4 GRA N3, N5. Good example: HSC_D4. in centralized, 8 files are showing as SENT (as per PIVOT) April 17.14. There were actually 18 sent in CSV on this date. 3 of them, have record status SENT in centralized yet they were not sent to CSV on April 17. On April 23, Judy able to include the sending of 2 of the ones already marked SENT in centralized April 17, but she had no complaint of a ERROR message that we heard of. 2 patients 10669 & 10656 were sent CSV but date SENT remains in centralized as April 17. One record 10634, is marked as sent in centralized but record status field on her laptop is incomplete. So far not able to figure out issue in regards to how she could these 2 without error message. We will monitor this weeks send and see if we can clue in further.-Trish Ostryzniuk 20:03, 2014 April 30 (CDT)

    • April 30.14: HSC H4: Pat Stein - error message about missing Minimal Data Set first send attempt today she had to enter fake admit "times". She said that this was not an issue for some time but the problem is back again.
      • Second issue she had today when going in and out of patient list, some of the patient with LOCATION HSC MICU were showing up on her laptop. When she would get out of patient list then go back in, LOCATION HSC MICU was not showing. Don't understand this one.
      • Pat also had a patient that record status field was set to SENT in centralized on April 26.14 at 1436, but on laptop record status remains incomplete because it is a MR pt. She could not send. I had to go into centralized and reset record status so she could send.
    • April 30.14: HSC_D4: Judy Kublick - could not send because serial 10657 record status was set to SENT on April 15.14 1803 hrs, yet on laptop, record status is Incomplete and for MR. Made the change in centralized to record status field so she could sent.Trish Ostryzniuk 20:09, 2014 April 30 (CDT)
  6. L_Log - we don't understand the logic why there is a difference "counts" when you sort and filter and don't sort and filter. For For example, we have to sort record status field first before we filter, otherwise not counting all the same way. Example: filter by vetted without sorting this field first and counts are different. Review with Pagasa. Both Julie and Trish getting error because we don't know why you have to sort before you filter. Pagasa only one that know this trick. Why should here be this trick. I guess we don't understand what Access is doing or we expect it should be doing something regardless doing it in this order.Trish Ostryzniuk 20:21, 2014 April 30 (CDT)

  1. Process for editing between send weeks? How to manage master centralized.mdb and Front end and PHI.mdb each week.
  2. Edit process in new database if accidently deleted field or file when editing master mdbs in main office: back up before Edit, then back up after edits complete? Back up to which location?
  3. Record Status field: issue of it getting changed in centralzied somehow during sending process from INCOMPLETE to SENT. Not resolved Trish Ostryzniuk 19:55, 2014 April 25 (CDT)
  4. import part of iTISS that corresponds to specific Tasks into centralized. See Tasks vs iTISS for mapping.
