Purpose

To identify the "sending service" or "originating service" for those who came to ICU from a prior inpatient location (refer to Task Meeting Minutes April 13,2015). This is different to the "service of the attending" during the patient's stay in ICU.

Data Collection Instructions

For each critical care' patient who were a prior inpatient before coming to ICU, add a tmp entry with the following:

  • project: sending service
  • item: one of the following:
    • Cardiology, Medicine, Surgery, Neuro, Family Med, Ob/Gyne, etc
    • If the patient comes from a temporary location like ER, Cath lab, other procedure suite, etc and was an inpatient in an acute care institution before that temporary location and the service is known, indicate the entry in the item.
    • STB_Critical_Care_Collection_Guide#Patient_origins_and_types might help with list
  • the other tmp fields are not used.

Start/End Dates

  • Start: not yet determined
  • End: there is no planned end date

Template:CCMDB Data Integrity Checks

  • each critical care pt has exactly one
    • Template:Discussion we should pre-populate the tmp line with an unacceptable value a la Postal Code.
