  This is just a template for starters.  Meeting for education plan is Friday Nov 30.12 @ 1030 hrs.

This category contains basic information about collecting Therapeutic Intervention Scoring System (TISS28) data

  • As of January 1, 2013 @ 0001 hours a NEW TISS28 form will be started for all patients who are in the ICU in the Region.
    • Any patient who is admitted, or is still in the ICU unit as of Dec 31.12 2400 hours, the bedside nurse must complete the old TISS form for their patient.
  • distribution process: Data collectors will be mailed packages of new TISS form between Dec 7-12th. They will be required to communicate with site manager, and unit clerk in regards to NEW TISS form to start for all patient Jan 1.13.
  • an education script & copy of final TISS form will be sent out by Basil Evan (QI officer)to managers at each unit. This email will then be distributed to ICU nursing staff, data collection and unit clerks in Region.
  • collectors will be required to assist completing some of the NEW TISS forms retrospectively during the transition period. We anticipate problems with this transition.
  • All patient admitted on or before DEC 31.12 midnight, will need old TISS76 forms filled out. Old completed form should be collected as usual and a new TISS28 form started for the calender day Jan 1.13
  • it is okay if TISS28 information overlaps on old TISS76 form and onto new TISS form for after Dec 31. Collectors will not have to cross out Jan 1 2, 3 etc if still done on old TISS76 form however, they will have start and NEW TISS28 form retro to Jan 1.13 for those that beside missed starting.
  • TISS form will be mail to Pagasa same way as before.

  • some collectors away over Xmas, and 3 stats during week during this period hence need assistance from all sites for this transition. Would have another collector at site check.

Forms will be print up at HSC printshop before 7.12 and mailed to sited by main office data processor (same way we do now. Tentative at the moment.

Who fills out the TISS

  • All shift - bedside nurses.
    • It was noted at a ICU Task Group meeting 2011-12-09 that having only nights staff entering the TISS is a huge problem because the majority of patients are discharged from the unit during the day shift. Also there are short stay patient being admitted for only a period of time during the day can be missed that way. The task team has recommend that all shift participate in completing the TISS form on their shift for the calender date. (Laura K). In October 2012, Trish provided Jodi with stats that showed the % of discharges during days shift, which also represent the high number of TISS dates not done because of the misunderstanding that it is night shift job only.Trish Ostryzniuk 22:01, 2012 November 29 (EST)
  • If a patient is in the ICU a very short time, a TISS score is required to reflect nursing efforts during this time.
  • Do not sent TISS form to the ward with the chart. Please hand it in to the Unit Clerk who will file it in the Research Forms binder at the front desk.
  • Once a page of TISS is completed, hand in the completed form to the Unit Clerk and start a new form. Please do not staple together. The scanning of the forms are affected if stapled.
  • TISS is to be done for a patients entire length of stay in ICU.
  • If a patient is transferred to another ICU, do not sent the form to the other unit or hospital. A new form is started when patient admitted to your ICU.
  • If a patient is transferred to another ICU in your center and the Service looking after the patient changes, then a new TISS form is required. E.G., a patient who goes from MICU under Medical Service and is transferred to SICU under Surgical service.

site compliance issue

  • collector have good relationship with staff in unit they work in. They will continue to provide feedback to staff on as needed basis.
  • data collectors should discuss compliance issue to be emailed to ICU manager.

  • current staff. Perm, PT & causal, nursing and unit clerks.
  • New staff hired (should be part of orientation)
  • ICU program - include in cirriculum
