Well, I have reflected on this and was making sure that I’m not being stubborn. But with the current newbies using the integrator we do need the log to track the accept/arrive times cause the integrator doesn’t populate both fields (cognos does). We also use the logs to check when Pagasa has questions about previous patients, and we do check to see if we have duplicated entries by looking at the log books. (sometimes the files are sent so we just look back at the logs) Also, I’m not certain I entered a patient I refer back to them. I think that once Cognos is really reliable we may be able to get rid of them but for now I think we should keep them.

Getting back to this question now. The [[Previous entry form]] makes available some information about previously entered and deleted patients - no names, but chart numbers are in there. Would that be good enough to replace the paper form?
