* We have discussed this at TASK in 2019  here is the final decision that was made - (Long discussion about if/how to split up the different types of beds at Riverview and Deer Lodge. Specifically, there are PCH beds and non-PCH beds. At Riverview the latter include chronic vent beds, rehab beds, and palliative care beds. After discussion we agreed that the only distinction we will have in those 2 places will be between the PCH beds and all other non-PCH beds, which we will call “Chronic Health Facility” beds. Likewise, St Amant, Selkirk Mental Health Centre and hospices will be considered to be Chronic Health Facility beds. **is this being asked in relation to pre-acute living situation, previous location and dispo?  I used this before in the overstay project where I would like to know who are already in PCH setting or not.  Since this project has been discontinued I no longer crosscheck the distinction regarding  Riverview and Deer lodge as a chronic health facility or PCH.  However, when there is a request asking where patient came from or where patient went after discharge, usually HOME/PCH are combined or grouped as one category.  --[[User:JMojica|JMojica]] 11:19, 2022 October 21 (CDT)

Transitional Care is provided in Transition Care Units (TCU)s.

A transitional care facility is a temporary place for patients that are waiting for placement into an alternate care setting.

Other medical care and rehabilitation is provided before going home, to a nursing home or to other more permanent placement. This includes Geriatric Rehab.

In the WRHA setting TCUs usually have a defined period of time for the resident's stay to determine most appropriate final discharge destination.

    • The only TC unit dispo locations currently available are for Misericordia and VGH. Deer Lodge as a dispo could indicate either PCH, chronic care, or rehab units. There is only an OAK ward as dispo option and could mean any number of different services including geri rehab. Same for Riverview dispo location could indicate PCH, palliative care, chronic care, or rehab. Is additional detail desired or needed for dispo locations to include TCU's? Should this be discussed at task? Thank you

Geriatric Rehabilitation Units

  • Deer Lodge Centre (DLC),
  • Seven Oaks General Hospital (SOGH),
  • Victoria General Hospital (VGH)
    • These inpatient services are generally focused on patients age 65 or older who require rehabilitation in order to return to their baseline function

Geriatric Rehabilitation beds are accessed through the Central Waitlist Coordination process.

Located at Health Sciences Centre (HSC) and Riverview Health Centre (RHC). HSC inpatient beds include:

  • Amputee Services
  • Amputee Inpt Program HSC
  • Spinal Cord Services
  • Neuromusculoskeletal Services

RHC (Riverview Health Centre) inpatient beds include:

  • Stroke Services
  • Acquired Brain Injury Services

  • This is not part of the regular quarter reports. However, maybe asked in specific request where the interest is the location the patient went after discharged.
