Data Collection
For any new medicine record added to the laptop after the project start date, capture whether there were any allied health consults.
Data Entry Instructions
The following lines will automatically be added to each new medicine record:
- Project: AHC_HC - Item: "not entered" (because you haven't set a value yet)
- Project: AHC_OT - Item: "not entered"
- Project: AHC_PT - Item: "not entered"
- Project: AHC_SW - Item: "not entered"
- Item: change the item dropdown as follows as you come across information
- "yes" - if you found any consult for (Home care HC, Occupational therapy OT, Physical therapy PT, Social Work SW)
- "no" - if you have finished the chart and didn't find one
- Date: date of the first consult if available (best guess better than nothing but not mandatory)
- Time: date (if available) of the first consult
- Integer: not used
- Real: not used
- Checkbox: not used
- Comment: not used
