This score is derived from the individual ADL entries collected under ADL General Collection Information.

How is this derived
What we collect
We collect the following components:
Scoring
The individual component fields are stored as their full words rather than scores, e.g. "ADL_Bathing" might have a value of "unassisted". Filling of the fields is driven by S_ADLOptions table which includes a "sorter" column to make the options show in the desired order in dropdowns, and an "ADL_Score" column that contains the score for the element.
Performance of each of the activities is evaluated as unassisted (0 points), minor assistance (3 points), or major assistance (6 points).
The ADL Score is the sum of points of all the six activities.
Data Use
- ALERT Scale Calculation/ALERT Scale
- Directors Quarterly and Annual Report (Medicine)
- Scale for each item : 0 - Independent, 3 - Minor Assistance, 6 - Major Assistance. Get the Sum of the 6 ADLs and assign to each category listed below.
- Frequencies by Categories (e.g. 1-6, 7-12, 13-18, 19-24, 25-30, 31-36)
