Date and Time the patient is discharged.

Collection instructions

The discharge date and time is the time written in the unit logbook stating when the patient left the unit. If this is not available, use the nursing note.

We track Discharge date and time for patients as part of our Minimal Data Set, i.e. this his high-priority data that needs to be collected ASAP.

discharge at midnight

If the patient is discharged at 24:00/midnight, use 23:59 hours as the discharge time.

Expired

Use time of death as discharge date and time. However, for Organ Donors patients see that article.
