*I agree - I made the change in the definition of Direct admit as already an 'in-patient'. However, EMIPs in other facilities are not in our down list and we don't want to add them. Therefore we will miss them in the database but that is fine because transfers from ER to ER to ER will create more confusions/questions. Let us simplify the process and follow the Special Case guideline above - enter Previous Location as simply ER (not ER direct). --[[User:JMojica|JMojica]] 09:53, 2017 October 13 (CDT) | *I agree - I made the change in the definition of Direct admit as already an 'in-patient'. collection instructions
for patient arriving at a ward/unit via parked in ER
Into Previous Location field, enter
- HSC - ER (direct)
- GRA - ER (direct)
- CON - ER (direct)
- OAK - ER (direct)
- STB - ER (direct)
Into Pre-admit Inpatient Institution field, enter the other site/unit/ward (including the HSC/STB/GRA EMIPs) they were last sent from .
Into Previous Service field, enter the service of the sending site if available, otherwise enter Unknown.
Special Case: Pt from other site ER as direct admit but never admitted at that site
Pt from other site ER, never admitted at that site, sent to your ward/unit as direct admit. Code as:
- Previous Location field: <your> - ER
- Previous Service field: Emergency Medicine
- Pre-admit Inpatient Institution field: NA / not applicable
We are aware that the previous ER would not be reflected in our collection.
example
Pt was examined & treated in the Selkirk ER (but not admitted as an inpatient) & then sent as a direct admit to HSC but stopped in HSC ER & was treated there till a ward bed was ready. Code as
- Previous Location field: HSC ER
- Previous Service field: Emergency Medicine
- Pre-admit Inpatient Institution field: NA / not applicable
Template:Discussion My understanding of "direct" admissions is that they must be an inpatient on a ward or unit or an EMIP in another facility. A pt sent from one ER to another ER that was never assessed by medicine or ICU would not be a direct admission. If they send a pt from, for example, Selkirk ER, the patient would still have to be accepted by either medicine or ICU once they arrive in the accepting ER. This would just be a standard admission (not a direct admission). If the patient was an EMIP in Selkirk (already assessed and accepted by their medicine service) and then transferred to a different hospital ER, then the previous location would be Selkirk EMIP, the previous service would be medicine and the pre admit inpatient location would be Selkirk EMIP.--LKolesar 08:55, 2017 October 13 (CDT)
- I agree - I made the change in the definition of Direct admit as already an 'in-patient'. --JMojica 09:53, 2017 October 13 (CDT)
