We currently have a number of versions of the data collection log forms that staff are using. The Data collection log form is a paper form that the data collectors use to write down some info about their patients to keep track of them. Much of the information is identical to what is displayed in CCMDB.accdbs Patient List and Previous entry form.

Data Collector Use
Keeping track of Serial number / Pat_ID
We now have Facilitated Management of Serial numbers. This eliminates the function of the paper forms to keep track of serials.
