General Information for the Chronic Health Facility entry in Pre acute living situation field.

Use the "chronic health" entry for patients coming from any part of

  • Deer Lodge (rehab) - except PCH part, code that as PCH
  • Riverview (palliative care, rehab, LTV)- except PCH part, code that as PCH
  • If a patient is discharged to a PCH bed at Deer Lodge or Riverview, then the option "Winnipeg PCH" should be used in the Dispo field.
  • These three need to become consistent and non-repetitive so they can't get out of synch. Ttenbergen 14:21, 2024 October 16 (CDT)
