Query Form Cognos2 Service Starter latest previous entry finder

Used by the "add patient" functionality in CSS to find the latest previous Pat_ID that uses the current CSS line's chart number, to then copy that profile's data. It matches on Arrive DtTm, Accept DtTm and Visit Admit DtTm.
