H4H is a high observation (HOBS) ward at HSC.

HOBs is an independent unit and not incorporated within a CTU staffing model.

Population types

The intention of HOBs is for patients that require more intense nursing care for part of their hospitalization. Some of the patient populations that are or could be admitted to HOBs but not limited to: Stroke 25, Post-Transplant Recipients, Care plan treatments/medication requiring cardiac monitoring or more frequent nursing interventions.

Workload splitting HSC_H4H

Starting Nov 26.14 this site is collected under a work load sharing name. Workload is divided up among the data collectors as follows:

  • A4 / p:Con Marks' laptop:
    • HSC H4H_a
    • All A MEDICINE patients and all NEUROLOGY SERVICE.
  • B3/D5 / p:Iris Deleon’s laptop:
    • HSC H4H_b
    • All RENAL TRANSPLANT patients, patients transferred from B3 & D5.
  • D4 / p:Michelle Lagadi’s laptop:
    • HSC H4H_d
    • All D MEDICINE patients.
  • H4 / p:Valerie Penner’s laptop:
    • HSC H4H_h
    • All H MEDICINE patients.
