Use the "chronic health" entry for patients coming from any part of

• Deer Lodge (rehab) - except PCH part, code that as PCH
• Riverview (palliative care, rehab, LTV)- except PCH part, code that as PCH
• If a patient is discharged to a PCH bed at Deer Lodge or Riverview, then the option "Winnipeg PCH" should be used in the Dispo field.
