2016 Time and Place changes

Our previous system of collecting admit and discharge information was convoluted because it has slowly evolved and never really been updated since the start of collection. We looked at all the fields involved (incl. concepts like "AMA") and implemented a new system 2016-07-01 (according to start time of Accept DtTm field). Many of the fields will draw from the S dispo table.
