There was a previous attempt to address some of this in Care levels in the community; this page and it need to be consistent, and consistently linked from the relevant field definitions. If we can get it short enough we might make a template to apply the instructions to each of the field pages. |
How does Chronic Health Facility fit into this? |
There was talk about comparing Postal Codes to known PCH Postal Codes. Since these might Include other buildings at the same site that are not PCHs, this check can at best be a soft check. Please add the list of these postal codes here. |
Background
Julie found cases where the pre acute living situation for direct admits are not consistent with Previous Location and/or Dispo. We want the DC to be aware that the pre acute living situation has a relationship with the Previous Location or Dispospecifically for PCH cases.
Examples:
- pre acute living situation = assisted living; Previous Location = Winnipeg PCH
- pre acute living situation = supportive home; Previous Location = St Amant
I rely on the combination of pre acute living situation; Previous Location; Postal Code and Dispo fields to identified new or already PCH residents prior to admission. Data collectors must understand that Assisted Living and Supportive Housing are not PCH and must be entered as simply HOME as confirmed by Dr Roberts.
As per email from Julie:
I have asked Pagasa to audit some records where the postal code matches with the PCH Facility. In most of these cases, Pagasa found that EPR addresses (no and street) match with the PCH facility’s addresses but the Pre-acute living situation =’Apartment’ instead of ‘Personal Care Home’ . this is possible because the address has the same format ‘Suite# - No and Street Name’ . PCH resident data prior to admission is not only being used in Medicine but also needed in Critical Care. Tina, is it possible to ask at entry level, say, – the postal code matches with a PCH facility (show the PCH address) but the pre-living situation entered is not ‘Personal Care Home’ , is this correct? I suppose DCs are getting the postal code from EPR where the exact address is also visible.
Pre acute living situation field and Direct Admits in Previous Location field
- Patients whose residence includes Apartment, Assisted Living,Supportive Housing,Prison / Jail / Correctional Institution or House and who were admitted directly from any of these residences must enter 'Home' in Previous Location field
- Patients whose residence is Personal Care Home and who were admitted directly from PCH must enter the corresponding PCH names in Previous Location field. Within Winnipeg RHA, there are 39 Personal Care Homes with their addresses and postal code specified.
- Patients from Chronic Health Facility and who were admitted directly from CHF must enter the appropriate CHF name in Previous Location field .

- Patients who are already a PCH resident when discharge out of hospital must enter in the Dispo field the same PCH location as in the Previous Location field.
- Patients from Chronic Health Facility when discharge out of hospital must enter in the Dispo field the same CHF location as in Previous Location field unless there is a changes in the level of care and needs to go to a PCH.
Pre acute living situation field, and Postal Code field
M:\PostalCode|PCH.xlsx shows the 39 PCH within WRHA region (http://www.wrha.mb.ca/ltc/pch/index.php). The excel file contains the facility name, the postal code, address - no and street, the number of PCH beds and other programs like Supportive Housing, Rehab, Palliative, Respite care, Adult Day programs. I added a column indicating the facility which is solely a PCH . Is it possible to use this table to crosscheck the Postal Code field if matching the postal code of 100% PCH facility and then crosscheck the Pre acute living situation field if entry = Personal Care Home. example of message - the Postal Code matches with a PCH facility postal code with address = no & street, is this correct? if address is exactly the same, then enter Personal Care Home into the Pre acute living situation field.
All the above crosschecks primary aim to determine those who are already PCH residents and therefore should not overstay in the unit when already discharge ready.
