*AG REPLY --- for some things it'll be quite clear, e.g. extraction of a tooth.  HOWEVER, for resection/removal of a tumor or mass, "extraction" is the wrong item in CCI.  Instead it should be one of these:  [[Excision, Resection, partial]], [[Excision, Resection, total]], [[Excision, Resection, radical]].  This is now added to the text.}}
