Additional Info

  • This code includes:
    • Those who are newly panelled and are waiting to go to a Personal Care Home, Chronic Health Facility or other long-term care medical facility.
    • This includes those that have a Pre-admit Inpatient Institution of PCH but are not discharged back to their original PCH, but to a new PCH. This is rare but it has happened
    • Those living in Community Facility with support.
  • Use this code regardless of whether the chronic facility or PCH they're waiting for is in or out of Winnipeg.

Excludes

  • These should be made consistent with Template:PCH Riverview Deer Lodge and should probably use that template to enforce consistent use. Ttenbergen 14:23, 2024 October 16 (CDT)
    • and I have lost track of the details Ttenbergen 10:45, 2024 November 1 (CDT)
      • There might be an easy way out of this question. We now only have one awaiting code; if waiting for any of them qualifies for this code then we may not need to know about the level of care. But then again, some of those sites also have acute/LAU type settings so we may need to figure this out after all...Ttenbergen 20:27, 2024 December 11 (CST)

Guidelines for the use of "awaiting" code

How much of a delay is required before we code "awaiting"?

Code awaiting if Dispo DtTm is not on the same calendar day as the Transfer Ready date and time. We realize this is different from Transfer Delay. For example, if patient was transfer ready on Tuesday, but left on Wednesday or thereafter, use one of the codes in Category:Awaiting/delayed transfer.

status changes and is no longer awaiting

If the transfer is cancelled do NOT remove the "awaiting" code.

status changes and is awaiting different destination

Patient arrives on unit already awaiting care

For patients who arrive "transfer ready" (usually to a low acuity ward), use one of the "awaiting" codes as primary if there are no active medical problems. If the patient is still actively being treated for something, make that diagnosis primary and the "awaiting" code a lower priority.

Possible reasons for delay that we can actually code
