Criteria

Must meet all of #1, #2 and #3A or #3B

1. Confirmed bloodstream infection in patient with a central line which has been in place >2 calendar days as of when the infection occurred.

  • e.g: if inserted Tuesday, first day that that CLI can cause a CLI-BSI is Thursday.
  • It can occur on the day of CLI removal or the day following CLI removal IF it had been in place already for >2 calendar days before removal.


2. Definition of a central line = intravascular catheter terminates at or close to the heart, or in one of the great vessels. It was used for infusion, blood sampling, or hemodynamic monitoring. It can be temporary, or long-term/permanent.

  • The following are not central lines: arterial catheter, A-V fistula, ECMO, balloon pump, VAD, peripheral iv’s, any device without a lumen (e.g. pacemaker wires).
  • The following are central lines: regular untunnelled central line, tunnelled central line (including port-a-cath, Hickman, Broviac), vascath, PICC, vascath, introducer


3. Confirmed means either 3A or 3B are true:

  • 3A. A recognized pathogen NOT related to infection at another site (such as UTI, pneumonia, surgical site infection) is isolated from one or more blood cultures
    • Recognized pathogens are everything except the following:
      • diptheroids (e.g: Corynebacterium)
      • Bacillus species
      • Propionibacterium species
      • Coagulase-negative staph (including S. epidermidis)
      • Strep viridans group
      • Aerococcus species
      • Micrococcus species


  • 3B. Patient has at least one of: T>38.0, chills, or hypotension PLUS any common commensal NOT related to infection at another site (such as UTI, pneumonia, surgical site infection) is cultured from TWO OR MORE blood cultures drawn separately from DIFFERENT SITES.
    • The common commensals are:
      • diptheroids (e.g: Corynebacterium)
      • Bacillus species
      • Propionibacterium species
      • Coagulase-negative staph (including S. epidermidis)
      • Strep viridans group
      • Aerococcus species
      • Micrococcus species


Attribution The Central Line-Related Bloodstream Infection to a Location

  • The infection is attributed to the location where the patient was on the date the infection became clinically evident......
  • EXCEPT if all elements of the infection are present on the day of transfer OR the next day, the infection is attributed to the location from which they were transferred.
