Collection Instruction
For each patient,
- enter the following option that most closely matches:
- Apartment
- Personal Care Home- including Riverview PCH and Deer Lodge (DLC) PCH
- House - also includes bungalow style condos (if specified)
- Chronic Health Facility- this includes patients that come from Deer Lodge (DLC) Rehab ward, Riverview Rehab, Riverview Palliative care, or Riverview LTV, St Amant, Selkirk Mental Health Center., Even if the patient was expected to return home, use this entry if this is where they were physically before the current admission
