    • b.Hard Copy (printed)- Annual & Quarterly - all sites report. ICU 2- copies: Dr. Dan Roberts
  3. Electronic - Director's Chart Report (MICU/SICU's only) - all sites report

The report mailing list is kept in Pagasa Torres Contact List on her computer. The contact list is exported every time there is changes to the follow location: X:\CCMDB_ADMIN_DATABASE\CONTACT_LIST\CONTACT_EXPORT_from_Pagasa
